It's usually the actors, sometimes execs. Dredd is a good example. The Stallone movie was originally going to star Schwarzenegger but he refused to wear something that covered his face. Stallone said he didn't have a problem with that so he was hired. Then the studio made him show his face more because he was a box office draw.
