Not really Sup Forums, but there was this really memorably bad moment in Vampire the Bloodlines video game. It's writing is really great, but at one point they forgot that they are writing a diary, and not an audio recording.
So a victim of a murder writes down her panicking, desperate pleas for help
IN HER FUCKING DIARY, WHILE BEING MURDERED

I hate when instead of just saying what the issue or secret is, the hero has to follow a bunch of cryptic clues to found out the secret or truth of something.
