Well the 'big one' is about what it means to be human - if you don't have free choice the you're not a man, you're a robot, you're controlled by those that can control.
If Alex was faking the success of his tretment, then the film wasn't actally exploring the idea of rehabiitation at all - it doesn't look at whether it is better for someone to have their own free will (i.e may or may not be rehabilitated) or for someone to be controlled for the 'good of society' (i.e will not be rehabilitated, but can pose no harm to society).

If you misunderstand the ending sequence you could walk away with that impression. By having Alex not be a drug addled zombie the last sequence is a sign of hope for his future, as it's a pretty clear marriage metaphor and shows that Alex has desire to better his life. The dialogue between him and the minister is also a reference back to earlier in the film where he lies to Deltoid; meaning he plans to betray the minister and expose his political scheme to free up space in prisons for political opponents, something the minister practically admits to earlier in the film.

It's only if you don't pay close attention to ACO that its themes feel like they're not being well presented, but they're incredibly well presented. It just doesn't hold your hand at all.

The movie is pretty straightforward. There are a few anomalies and questions that were purposefully designed to have no answer. There is nothing to really "get." Art is open to interpretation.
