In the book it's fairly ambiguous, and the movie was supposed to be the same way but failed by slapping the viewer with all the mental illness shit making it seem like a plot twist/reveal instead of another factor. The message from the creators of both the book and the movie is that he was definetly whacked out, and that some if it is hallucinatory, but that he certainly killed several people.
