Clone Wars never truly ended. After CIS leadership was struck down on Mustafar, hundreds of worlds still kept resisting the "peace" that was forced on them by the new Galactic Empire. Palpatine still kept large military forces in various pacification campaigns all over the galaxy, with some planets still remaining active war zones for years if not decades.
Eventually some of these resisting CIS words or sub-factions became Rebel Alliance founding cells/members.
Clones having faster growth also had been programmed not to live longer than 15-20 years, meaning most of the just kept dying in the various pacification campaigns/Imperial projects until maybe some of them just died of "old age".

The ones that obeyed order 66 became stormtroopers and they were slowly phased out of Imperial ranks due to death in combat and old age (the clones aged twice the rate of normal humans). Some of the most experienced clones, like Commander Cody, were kept as instructors for stormtrooper conscripts. The clones that disobeyed order 66 went on to do various different things from joining the rebellion (Captain Rex) to joining a gang of bounty hunters after being frozen for 50 years (Kix was a clone who knew about order 66 during the clone wars, before he could tell anyone he was captured by the separatists to be taken to Count Dooku to be questioned about order 66. He was frozen in cryo sleep and the ship he was being transported in was shot down, where it stayed for decades until a bounty hunter gang, led by pic related, found Kix in the ship and let him join their gang. If you go by the old canon the remainder of the clones by the time the empire rolls around are remade into vaders personal troops of which half were blown up in the deathstar and eventually they all go down fighting in hoth or some shit. But eventually they just get too old and stormtroopers were begun to be made with new clone templates and regular recruits.

In battlefront 2 the kaminoans make new ones but they get BTFO by the old guard.

The old guard are battle hardened with new equipment. The newer ones were basically fresh out of the vats with older stuff. Besides, it was just a few facilities, the empire could've glassed them from space.
