The answer is B. This is confusing to people because they're confusing the inertial reference frame. Let's define two reference frames, the inertial frame and the body fixed frame. To not break the rules of Portal, portals cannot be placed on accelerating surfaces. So this means the surface that the portal is on is stationary/inertial.
The other frame, the body fixed frame, is fixed to the box. In the question setup, the observer is viewing the motion relative to the body fixed frame, so the inertial frame appears to be moving. Instead, imagine that in the first part, you are an observer in the inertial frame, so the box is moving towards the portal. Thinking this way makes it obvious when you look at B that the box would shoot out of the portal.
The confusion comes from two issues: 1) relative motion is not intuitive when you do not define reference frames. 2) changing perspective and reference frames is very confusing when you do not keep track of your reference frames.
