Iron Man's super hero name derives from his suit. His costume is literally an Iron Man - and that's what he's known for.

Underoos is Stark's giving Spidey a nickname in the same format as his.

I see it as a nice, almost endearing touch that gives them a mentor/sidekick relationship. Like Batman (a winged flying creature) and Robin.
