Remember this was a guy that could easily pick-up young guys at gay bars to have one night stands if he was really ugly he would haven't been able to pull it off as easily unless he paid them.
I assume his weird behavior is what made him unpopular at high school not his natural looks, maybe John Backderf remembers him as an uglier guy than he really was, or he drew him as an ugly person because the average reader wouldn't buy an average looking kid could be a psycho.

Take it with a grain of salt, but Backderf's style on that book didn't let that much liberty for "handsomeness". Still, his tard acting was his meal ticket for martyrdom/popularity as the school's freakshow.
