The blood looks real (most amateurs make it scarlet red which doesn't look like a real injury at all), then again the guy is a horror movie maker so who knows.
However, 1) the amount of blood and location of the injury suggest a small artery was hit (scalp injuries bleed a lot), but the blood is coagulated with no active bleeding. While it is very possible the bleeding stops on its own after a while, it looks like the picture was taken right after the fight (at least before the emergency team arrived or he went to the ER), and it's very unlikely an artery just stops bleeding on its own so quickly without even a compression.
2) the blood on the external side under his left eye (right side of the picture) doesn't make much sense, it looks like the blood went down to his nose then went left (why not) but also up, which is very unlikely (gravity and all that)
It's not very conclusive but I have doubts. Then again, even if it's real, it still doesn't prove this has anything to do with the election, or that he didn't start the fight.
