No, there is no Constitutional requirement that voters be able to read. And as I understand it, those who are unable to read (or are blind, or whatever) are required to have accommodations made so that they are not unfairly deprived of the right to vote. And literacy questions have a long history of being abused - how would you ensure that whoever is scoring them isn't unfairly discriminating against certain groups?

You're right about literacy not being required to vote, but if it were, there would have to some way to test it objectively. Maybe poll workers could just give standardized tests with basic grammar and "correct this mistake" spelling questions. No human grading would be involved. It would take like ten minutes to complete, and if you get above say 70%, you get a ballot.
