There's a ton of synthetic opioids, mainly fentanyl derivatives, being used as cutting agents to give the seller a wider profit margin.
The trouble is that it's not possible for people without pharmaceutical manufacturing equipment to mix the drug safely in powder/solid form.
The only safe way for the ordinary person to take such a drug would be to measure a very small quantity with an expensive, accurate scale, then to dissolve that in a known quantity of water to achieve a solution.
Otherwise you inevitably get 'hot' doses that will kill people.
Whenever you hear about dozens of people in the same town all ODing in one night or within the span of a couple days, it's because their stuff came from the same place and was adulterated with a fent analogue.

If you deal heroin on the street, chances are you're not the most ethical person in the world for starters, so it's not a hell of a leap to start diluting your drugs with deadly opioids to make more money.
