If anyone is going to possibly launch a nuke or two, it's going to be the Norks before they get toppled. Assuming the leaders of NK don't just try to leave with some warheads to sell on the black market.
The deaths will be simply due to a population of Norks committing suicide or starving or otherwise from the fact that their entire world has died and now they have to join with the rest of the world.
For immediate casualties, you have to go for NK doing as much damage to SK as possible. No way NK can win against US and SK, so it's best for NK's final moments to just do alot of damage. So if any Nukes, expect one for SK and one for Japan. They could attempt to fire one towards the West US coast, but you'd have to gamble on if Nork Tech can even make it that far over the ocean, and even then, if the US doesn't just attempt to take out the one or two missiles coming to America. It's just a few missiles, not a barrage of nuclear hellfire such was the fear about Russian nukes.

In the past yes. But eventually China realized "what's the point of supporting these starving, brainwashed peasants that just keep doing stupid shit to attention from the world? What do WE really get out of this?".
Hence in recent times, China hasn't been really giving much of a shit for NK. If anything the only thing that China would have to worry about, would be the refugees flooding the North-Eastern Chinese border.
