My opinion is that there are no such things as rights, so while a group of people might choose to give it's members healthcare, they might just as well choose not to.
Other than that, one could argue (playing devils advocate) that there are sicknesses (mostly genetic ones, but also some mental disorders and STDs) where treatment actually causes more people to have that sickness, eventually hurting more people than if you'd just left the initial patient to die or kill himself unless he can prove his worth to the whole group by means of either aquiring money for treatment or finding somebody else to pay for him.
