Am I crazy to think that these countries are sinking and firing at each others ships in acts of provocation and just use a cargo ship to blame it on?

Actually, from this photo, it looks like the AEGIS support hardware section is damaged and there's probable underwater damage to the bulkheads and hull from the freighter's bulbous bow.

At night, another vessel in a lane may confuse another helmsman's perception of a ship going to port or starboard and turn into another vessel on accident due to the illusion/assumption of the navigation lights of the first vessel.

In the US Navy there is almost always a court martial if a ship is disabled or sinks, even if the Commander is a hero.

For example, the commander of the USS Cole, even if he was credited for saving the ship by ordering the compartments sealed after getting some intel reports, still had a Court martial, even if by the end he was viewed as doing exactly the right thing.

One of the ships I was on had a female XO, we werent 10 minutes from the pier, could still see it, and she dragged the port side down a railroad bridge support piling.

Naval warships aren't merchant vessels. They are surprisingly maneuverable. They are fitted with CRP (controllable-reversible-pitch) propellers, which allows them to stop very rapidly compared to civvie ships. If it is properly designed, a ship is MOST maneuverable the faster it is moving, due to the increased water flow over control surfaces.

You would figure between radar, and port and starboard bridge watch, somebody would see a ship the size of a football stadium

Some WWII fighters did have shielding to protect fighters in dogfights. It would be pointless to so equip a fighter with armor these days as most engagements wouldn't approach gun range, and if it did a hit just about anywhere on the craft would put it out of the sky.

Doesn't the Destroyer have advanced sensors like radar and such? Kind of ironic that these ships are supposed to defend themselves from subs and anti-ship missiles, and a huge cargo ship sneaks up on them.

If I were to make a guess, I'd say there were in shipping lanes in a shallow part of the ocean where their drafts required them to stay in the channels less they risk grounding on a reef/sand bar/shallows and one drifted out of their lane into the other's lane or that while trying to create a greater distance between the two, almost drifted into the shallows and turned back in a little too quickly. It was also dark.

It's happened quite a few times in the narrow channel on the upbound route to the Soo Locks a few times. Shit, one freighter cut another in half and sunk her in a 2 mile-wide channel under the Mackinac Bridge back in the 60s, even with collision radar.

In today's navy, literally any fuck up for a high ranking officer is career suicide. Don't get me wrong, colluding with another ship is a momumental fuck up though. I was in one ship that collided with another during RAS. The CO had literally just taken over command, the accident happened, his career was over as he was relieved of command.

Life is unfair.

For example, the CO might of been asleep in his bunk and a Jr. officer had the watch, but it still the CO fault since the CO job is to make sure the people in charge are up to standards.

Also note, a court martial does not mean punishment, it just means a trial. The Court Martial can clear the commander of any wrong doing and move on.
