This is what I keep trying to tell people. Our culture is different and our population is huge. I would be more accepting of single payer if our population wasn't so massive, we weren't number 2 in the world for obesity, violent crime wasn't through the roof, our people weren't lazy as fuck, and we deported any illegal who would take advantage of the system without putting taxes back in to it. Also the super rich are incredibly good at hiding their money in offshore accounts and taking advantage of the tax code. There are so many things in our country that would make single payer extremely difficult to maintain. Do people even think about this shit before they start spouting off about how any other form of healthcare is murder?

The problems lie more with the hospitals and their suppliers than insurance companies, but both are really to blame.

If you provide a "minimum standard of care" in the form of some amount of money, the price of a GP visit will increase to just less than that amount.

If there is no upper limit to the amount single-payer will cover, or the amount a federally-regulated mandatory participation scheme will pay, you can rest assured that having a yearly checkup will soon become a multi-million dollar expense. As you can see single payer advocates suggest money could be saved from improved efficiency. Figures such as 150 billion reduced overhead are passed around. But as it stands 5 big insurance companies cover 80% of the market, I remain unconvinced that a government agency can work more efficiently than 5 highly competitive private companies.

However the bigger issue with single payer insurance is that it opens up our healthcare rolls to people who don't pay taxes or pay little enough that they de facto will not pay into the healthcare pot. As someone who pays into this pot already, heavily, with medicare and medicaid and hospital subsidies, I am uneager to expand our rolls to include 40 million more poor Americans.

It changes the bargaining structure. With single payer, the government is the only entity that buys healthcare, so it can demand lower prices. It's how medicare gets better deals than private insurance.

Our system is good for some thing and bad for others. If you need a super specialist fast, then the American model works well. If you need something basic, then Canada has a better model.

Titty cancer is a really interesting case study to compare the two models. Women diagnosed with titty cancer in Canada often come to the US for treatment because American doctors treat it much more aggressively. That helps some of them, but titty cancer screenings have a high rate of false positives and some types of titty cancer aren't particularly malignant. What that means is that American doctors are probably over treating lots of women who would have been fine. The American advantage is a slightly higher survival rate, but the Canadian advantage is less unnecessary treatment and less spending.

Insurance companies take 20%. Of the 80% that gets to hospitals they take 10% profit, 70% salary (lots of non-medical but hard to establish) and 20% medical supplies. Of the 20% of medical supplies about 50% is manufacturing/R&D, res is profit and advertising.

Of the % of salary that actually goes to medical personnel most of that salary is spent on paying them to fill out paperwork.

Once you do a systemic analysis about 10% of health care dollar actually goes to health care.

The medical system is shit, public hospitals are shit, we need to wait months for an appointment, we still need to get private healthcare on top of paying for the public one because it's still not enough, and even with that, with anything related to eyes, teeth and ears, you're on your own.

There have been some experiments at the state level. Vermont tried to establish a single payer system, called Green Mountain Care, but gave up because it would have required a huge tax hike. California has been looking at creating a state run universal healthcare system, but doesn't know how to pay for it because it would be bigger than the state's budget.

Nevada just passed a bill that would let people pay a premium to get on Medicaid. It's a promising idea, but I'm glad that I don't live in the experiment.
