Basically sites will approach network operators that have peering agreements with other operators and make deals for estimated traffic at fixed prices and performance service levels. Many of these operators have privately owned cables and networks that they run their traffic on. This allows hosts to get desired bandwidth and latency for delivery, and sites who don't use this use the public network (ISP owned) and have less bandwidth and worse latency generally.
The free market wasn't working, which is why ISPs had to be classified as Title II.

Owning so many exit nodes, as well as who knows how many relay nodes, makes it much, much easier to trace the path of an arbitrary packet to its source. Tor is not safe and hasn't been for years.
TOR is one layer of "security" you can use, relying on any one technology is doomed to fail.
