That's not how the timeline works in this series. It's more like "branching path" timelines, there are infinitely many timelines side by side occuring at the same time and each moment in time has infinitely many branching paths which all equally occur. Every timeline happens, and every timeline is no less real or fake than any other timeline.
That means that Grandma Shiho in Momo's timeline will be unchanged, since she's from a different timeline than the one which is currently occurring.

You just don't see "Sentence Titles" in English Literature, like you do with Japanese. So translators tend to avoid them, thus why all the official overseas releases either prominently use the Japanese short-hand as a title, or come up with a gimmick title.
