This whole thing was a giant PR campaign. They wanted to parade a big white guy around and have him beg for forgiveness, and condemn the US. Which they did. The entire spectacle was scripted.
They knew that eventually they would need to give Otto back. But they couldn't risk him coming back and telling everyone what really happened. That he was tortured into giving a scripted performance that he practiced over and over again.
They also couldn't just kill him. That would be too risky.
So, they did the most logical thing. They gave him brain damage so that he couldn't talk, and then handed him back over to the US just before he died.
This way Otto can't tell anyone what really happened, and he dies in US custody so that their hands are clean.
