The story of a mid/high school boy who likes dressing as a girl and a girl (his childhood friend) who likes dressing as a boy. Except it's played completely straight (ignore the pun) and treats the whole issue with the sort of tact and dignity that you simply wouldn't have thought possible from an anime series.
To give you an idea of how tactful the writing is, I think all cross dressers and trannies are degenerate mentally ill freaks and even then I STILL found Hourou Musuko to be a thought-provoking and emotional series.
