The department of defense has problems acknowledging they have systematic issues, even to themselves. Mostly seems related to their pigheadedness and a chronic lack of funding. The -specific- incident was the situation surrounding the death of 2 soldiers and 1 wounded when a grenade explosion occurred during training, turns out just about everything went wrong, the grenade was outside of the specs, had know defects, was stored incorrectly, the local hospital wasn't equipped to deal with treating that third soldier, even though parliament was given assurances by the minister beforehand that they all would be up to a certain standard. They initially mostly ignored the report, but after it became public the minister stepped down, as did the head army honcho.
