It'd be better if Diavolo was a woman and Doppio was her QT male alter-ego. It makes sense for her to try and make even her alternate self as detached from her actual self as possible, not to mention it'd be a lot easier for a woman to disguise herself as a cute boy rather than a grown-ass creepy man doing the same.
Araki is not a very good writer. Sometimes I wonder why I like this mediocre series, it's literally full of asspulls, plot holes and forgotten plot points

I don't understand why this artist doesnt realize that there are pompadour styles for women. Okuyasu also looks like he's from the 1800s, but I'm not really sure how attractive he's considered in the manga.

Diavolo isn't anywhere near as powerful as people here hype him up to be. The guy himself explains why he won't attack Buccellati's whole gang outright, because he knows that the short range of his stand is a weakness, and thus prefers sticking to the shadows and fucking with single opponents.
Not to mention that Doppio (who appears to be Diavolo's 'true' self, the original, given descriptions of him before he burned his town and went off the radar) is an unavoidable weakness, he nearly lost against Risotto.
