Junji ito isn't scary either though. Sure the stories are decent and interesting but scary? I wouldn't go that far. Mainly because this . Its especially hard in a manga / novel form.

Once I finished Ito's work I was hooked and then the cold realization came that there isn't other shit like it really. The best I can find is bizarre or interesting but not really spooky. And even that is hard to find without it being shit.

To be fair horror is hard to pull off in any medium. When was the last good horror movie? Or book? Stephen King is a meme, his books ae not that great. And with movies, everyone overuses jumpscares, completely missing the point of horror. Anime either goes jump scares, or does something so ridiculous that it's hard to take seriously. Like Another.

Horror in general is shit, especially if you want to be scared. Everyone just applies the same feeling they got the few times in their life they genuinely were scared to their expectations of how something labelled "horror" should make them feel, but getting that much of a rush from anything is rare.

pretty tough to do since any good VN is based on decision making, while horror is based around following characters that make piss-poor decisions.

Ito is not scary either, his works are mostly a hilarious black comedy with absurd body morphing and silly storylines.
Junji has unique ideas, but to me they lose their luster the further they go on. Take the fish one for example, interesting concept, however devolved into nothing more than shock horror with each page being more and more gruesome just for the sake of disgusting the readers rather than write something compelling

His stuff has some shock value but it becomes almost comedic after a bit.
Closest is psychological horror, "makes you think at night" stuff like Shiki, Texhnolyze, etc.

That's probably what the whole genre should be. A good horror makes you terrified after you watched/read it, not during with cheap effects. A good idea for a horror is to take something close to real life and make that scary, so when next time you see it, you will feel uneasy. The simpler the concept that gets corrupted, the better.
The thing with horror is that you have to have a lot of meaningful, dramatic buildup to draw people in. A lot of people simply forgot how to write or don't put a lot of effort into their writing. You have to slowly drag the audience (and the characters themselves) away from their comfort zones, into a situation where they have to adapt and survive.
Modern media has trouble doing this. It tries really hard to make the characters and the story socially acceptable (how many times does the slut die first). There's also the trend of meeting the audience's expectations ahead of time instead of trying something new. It's simply a bad time to be a horror writer, unless you're just doing it in your spare time and giving it away for free.

This is the literally the only anime in existence that actually has a good story with some interesting and chilling elements which doesn't rely on unnecessary gore or cute girls killing cute girls.
Junji understands the medium and designs to exploit the medium
Other horror authors/artists design the story before taking the medium in mind
It's also why adaptation of Junji's stuff is difficult because it's so tied to how manga is presented

It's not really "scary" either (I doubt any work of fiction is really scary if you're past 13 yo) but it can be really unsettling.
