Akio was a an overworked prince so his sister tried to save him. He became a spoiled brat because of this. Utena tries to save anthy but fails. Anthy is inspired by her, and finds the courage to leave. The end.

A long time ago Anthy and Dios were siblings of some ancient race that was like gods among people.

Dios decided he wanted to help everyone and become the perfect prince. For a time everything was good.
However as time went on this was doomed to crumble for two major reasons. The people were starting to take him for granted were abusive with their requests. At the same time Dios felt that he was changing quite possibly because he started getting interested in sex (note how in the shadow play he never gets kissed, quite possibly at some point some princess tried to go all the way with him), but also about power in general.

Now at some point Dios asked Anthy (or Anthy knew he wanted it, or Anthy just did it) to help him. This involved sealing away his pure heart as a princes. What was left was Akio, (un)fortunately Akio's own magic was tied to him being princes and he therefore lost most or all his power.

Akio without his heart becomes the bastard he is now, and starts fucking Anthy.
Akio is also aware that his heart could in theory be awoken any time by a princes' nobility, but he for many reasons no longer has his. So he has Anthy trick many boys and girls over the years with the dueling game. The end goal of which is to take their soul sword and unlock Dios' power.

Funny enough the manga implies that should he ever actually obtain Dios' power again the contradiction between who he was and who he is now would destroy him.
