The only state I've seen whose result wasn't in the Margin of Error was Wisconsin, and if you take a look at Wisconsin's polling in pic related, you see that there were only three polls conducted in the week or so before the election. Look at states like Florida and you'll see a lot more polls were conducted in that time frame.

Please look at this site which shows all the state polls, you'll see that the results aren't really that far off of what was predicted:

I should also add that Trump got less votes in Wisconsin than Romney even though he won. The polling may have been correct, but traditionally Democratic voters either decided to switch sides at the last minute, not vote at all, or vote third party.

B. A difference of 3.7% is hardly anything substantial.

The only real substantial differences in polling occurred in Wisconsin, Ohio, Iowa, and Utah. Furthermore, in states like Nevada the polling was actually IN FAVOR of Trump winning, even though the state went to Clinton.
