Because otaku romance stories are more about a fantasy of romance than actual romance. While actual romance stories (even the ones that are more aimed at guys, look at Seto's mangas and Good Ending wich both feature the MC going out with different girls before settling with one) don't care as much about it.
Now those stories can also have some level of fantasy, but I would say that's the biggest difference.
