Chances are that it's just inconsistency resulting from changing authorial intent. I really doubt Itagaki (or any other mangaka) imagines the skull of their character first. And if I recall right, the skeleton picture was just a cover page or something, so it wouldn't require it to be a canonical mess-up or visual effect or whatever.
