It did, in fact. The government did purposely add poisonous substances to alcohol, and this did result in thousands of deaths during Prohibition.
Regulations don’t solve this problem at all. Tainted alcohol is still being sold despite regulations. If you actually read about regulations, it’s more about big business stifling little guys from growing, while big business can poison you, but you can’t legally do anything about it because they’ve complied with said regulations.
Prosecution on the other hand works.

Murder/Manslaughter would still be a crime, so the server/owner would face punishment. Then, no person would buy from that bar ever again - and it would shut down.
And then their offspring buy a nuclear warhead with a loan from a sympathetic warlord, then detonate it on the bar as punishment for violating the NAP.
They then sell the bar owner's young children to rich pedophiles to cover the loan and balance is restored.
