No, most cities have businesses that powerwash a block or a few spots, places with a lot of homeless(Certain parts of San Francisco) often the stores themselves will have power washers for washing in front of their business.
It's a mix of issues. We could talk about the contributing factors all day, I personally have been "homeless" for years and I've got dozens of stories about a time I had to shit in some random city and there wasn't anywhere open or if there was I didn't have any money. I've walked miles at times, carrying my 75l backpack, just to shit.
The best solution I've noticed seems to be a combination of Portajohns any 24/7 bathrooms. 24/7 public bathrooms often turn to cesspool of Bullshit quick, no matter how high tech (San Francisco has some hilarious suicide booth-esque toilets that clean themselves every use) but the most efficient I've seen is the public restrooms on some of the beaches in California. Two rows back to back of 20 or so single person toilets with a few handicap stalls on the end. Most beaches outside LA have the riff raff contained though, so it's hard to say how well such a permanent system would function near a skid row or downtown situation like what Seattle, Portland, SF, LA and San Diego have.

When it became clear that Seattle was the next big city and there was no stopping it, the residents grudgingly obliged to the high rises as long as they got new sidewalks and new schools. After the 90s it was all over for the area.
