I'm fairly sure Touma will handle that or leave it to Accelerator. There's chances for things to fuck up if he and Accelerator don't stick together though. I could see Choronzon tempting/threatening Accelerator if Touma doesnt get to him first or just leaves him to it without communication

It would be pretty cool to see Aleister hanging out in Touma's dorm room and playing with Sphinx/bullying Othinus and fighting with Index for food. The other characters would have no idea this random loli is the head of AC.

Actually Touma doesn't need to keep an eye on Accelerator. Anrei is involved too so he can just trust Hamazura with making sure Accelerator doesn't get kidnapped, seduced by the demon or decide to do something else troublesome.

So do all of these Aleisters the same minus differences in age, gender and body weight? Like, one might be a chubby old woman while another is a sickly young boy, but if you sat them down side by side you'd see a freaky resemblance. Or is the appearance wildly different? Like one has red hair and another brown while another is blonde. Are all of these Aleisters aware they are Aleisters? If they all look wildly different and aren't aware they are Aleisters what's stopping most of the cast from being Aleisters?

So Laura gets into WB and fucks Aleister up with Choronzon; but this is all keikaku doori because there are now a billion different Aleisters.
Where do Aiwass and the Kamijou faction fit in all this?

Imagine breaker and Accelerator are needed to access the core of Aiwass, or something like that. So Laura and her goons will come after them.
Shiage is bound to get involved too since Aleister mentioned Aneri being a part of his countermeasures.

If NT really is going to be 27 volumes long like its biblical namesake, we are now between OT Volume 14 and 15 percentage-wise. It seems very likely that this is going to be a long climatic arc like GRS.
Also, 18 is two thirds or 66,6(6666...)% of 27.

where should i start watching this anime? aren't there like three different versions or spinoffs?

Touma has billions of years worth backed up so s/he's set.

Index is the main series, the LN adaptation has 2 seasons. You can choose to read the LN from the beginning or on volume 14 which is where the anime stopped. Most of the discussion in these threads are focused on the latest volume which came out today.
Railgun is a manga spinoff that also has 2 seasons, you should watch that too. Read the manga from the beginning, it's pretty good.
There are three other manga spinoffs (Accelerator, Idol Accelerator, and Astral Buddy) but those are better off being read last.

there is also a more confusing one but you can watch it like I said above.

The anime is confusing as fuck because of the amount of content it leaves out or cannot portray to the viewer correctly.

I read the novels along side the anime. It was a little easier that way.

Spinoffs are Railgun, Astral Buddy, and Accelerator.

Railgun anime is a little more true to the manga until you get the anime only shit added in. So keep that in mind.

Also the manga has several moments the anime excluded, like Touma fucking up Mikoto's investigation of Level Upper. (Which is the moment leading up to the prologue of the first novel)

Accelerator makes references to NT, but I'm not sure if its a big deal that you get that far or not.

Not sure if anything substantial has happened in Astral Buddy yet, but I hear its a lot of fun and I'm going to be reading it too.
