However, later that day Akko is invited by Croix to Wagandia; desperate to catch up with Chariot, Akko decides to ignore Ursula's warning and accepts.

Those are all appearances of Woodward. Her episode 16 version is very inconsistent with the rest. She is in what looks like a corporeal form, has no horns and no cross on her forehead. Granted, her picture from the book has no horns either, by it probably comes from a time she was still walking the Earth.
