I used to only sleep 4 hours a while back. It has to be a conscious decision, although you do acclimate pretty quickly, it definitely takes discipline.
Simply put, he is extremely goal oriented, and even without the sleep, this guy doesn't accomplish everything he sets out to do. So, he simply doesn't sleep much.
I believe it is perfectly safe/healthy, too. 4 hours is about a full REM cycle or two. Even if you are an athlete you don't even need any more sleep, although it may sometimes impede your progress.

He apparently drinks a lot of Diet Coke which has caffeine. He doesn't have coffee in the mornings apparently. People claim it's 12 cokes throughout the day but it's likely less.. but even then I'd say that gives him quite the caffeine boost. Maybe he fitted that into his diet because it keeps him going a bit. But also he has people who wait hand and foot for him and so there's a lot of labor alleviated from his everyday life. But he's President, and that isn't easy even if you had someone wiping your ass, so somehow or another he's still facing shit most of us can't even dream with only 4 hours of sleep while eating McDonald's at 71, and that's impressive as fuck.

I think it's to appeal to Americans. A President with a fit suit looks like he's doing better than the rest. Obama did the same shit after a while. Sometimes he wouldn't even have the suit jacket. Trump never did that, but he did make sure his suit isn't too imposing. It's imposing in a different way, where he's standing tall with loose suit and a tall red tie. I see what he's doing there.
You can find pics of Trump in a good suit. I think in his old age though he's becoming less and less of a mogul whore and just being himself, and that's pretty cool considering he's the President now anyways

First off, some people just sleep less. I spent most of my life sleeping only 5 hours a night. That's 2 full cycles of sleep, and I'm fine and refreshed the next morning.
Also, as you age, you need LESS sleep. 4 hours sleep for a 70 year old is fairly common. It was when he was only sleeping 4 hours when he was 30 that was remarkable.
He probably gets two full cycles of rest in his four hours.
Most brilliant people sleep less than the normal human. Their brains seem to cycle through sleep faster, more efficiently than the average human, letting them get the same result for less time.
