Japan doesn't distinguish between clinics and hospitals the way the US does. Most hospitals are very small and offer walk-in primary care services, which is where most people go for common ailments. It's kind of like urgent care, but far cheaper than an office visit in the US.

They also use the same word (病院) to describe anything from a small walk-in clinic with one or two doctors to 1000-bed tertiary care hospitals.

It's still bretty great, but the format has changed to look at the long term. A long term series with a clearly winning tomboy isn't the end of the world.
