italians consider it uncouth. you should twirl a little on your fork with out a spoon. then cut some meatball or other protein. stabbing the fork into it. so the meat holds the twirled pasta on.

The spoon is used to serve. In Japan many meals are shared among all at the table, so spaghetti would be delivered in a large dish and then a fork+spoon would be used to serve spaghetti into smaller dishes for each diner.
