You walk into a gun store, pick out whatever, and negotiate the price with the salesperson.
Then they make a copy of your driver's license (to check your age and identity), you fill out ATF Form 4473 (which is basically you saying "My name is user and I swear I am not barred from owning a firearm." - it's a legal document. If you lie they can jail you). Then they send your details to the background check system, either through the website or via phone call. After a few minutes (10-15) they get a response that says you're either clear (you can buy the gun), not clear (something on your record is preventing you), or "pending" (usually happens when you have a common name like John Smith and there are six other John Smiths in your state, all barred from owning a gun, and the background check takes a day or two to verify that you are the right John Smith - usually avoided by just giving them your social security number, but some people don't like to give out their SSN because muh identity theft).
After that you pay and walk out the door with your gun. In some states you have to come back a few days later while the store holds it for you (this is to prevent suicidal people from buying a gun on impulse and blowing their brains out in the parking lot). Sometimes the store staff will walk the gun out to your car (for a similar reason).
In most places you can buy a gun in as little as 20 minutes.

You can do whatever you want to it after you buy it. Most things people do to their guns are stuff that you can do at home with basic tools. Some more advanced stuff requires somebody with special knowledge and tools - a gunsmith, but there are a lot of these people around, especially on the internet (you ship them your gun, they do shit, they ship it back). Most shops do not actually do any work themselves on the guns they sell - they just take them out of the box they came in, stick them in the display case, and that's it. Some shops have a resident gunsmith or maybe a staff member who has a hardon for custom work and will fill the store with shit like that, but most gun stores are pretty basic.

You "customize" the gun with anything else you buy in the store like sights, extra/extended mags, etc. To buy ammo, you have to be the same age as required to buy the gun that shoots it (here in Virginia, you have to be 21 to buy a handgun, so you have to be 21 to buy handgun ammo). I bought a shotgun the day I turned 18 with my own money because my parents never owned guns and I've always believed every citizen should be armed.

Does anyone buy anything that way now? I get all my guns online I normally go to gunbroker or budsgunshop and browse around. Take the Ruger 10/22 for example there are probably more then 100 variations of it that you can order. Then after that you can do a lot of customization yourself or have a gunshop customize it.
