Escaping hell doesn't revive someone, the person is still dead. What revived Araragi was the ritual involving the Yumewatari. I don't remember if it was mentioned if Gaen could revive Hachikuji or not, but it was certainly not in her interest to do that.

That's due to the "porygon event" or whatever with potential seizure stuff. It affects tv broadcasts but not physical copies. Screen dimming in tv broadcasts is also noticable for explosions, and rapid changes to mostly white scenes. I am thinking of the Boku no Hero Academia OP right now, that had a few dimmed parts in it
