Mitsuha opened the door for Taki - but Miki already knew what he was like leading up to that point. Unless you weren't paying attention it was Taki "falling for" Mitsuha that "lost" Miki. The only reason why Taki didn't end up with Miki (all other things being equal) was that he wasn't interested anymore in her but was instead interested in Mitsuha. Miki just picked up on this before he (taki) did.

It was pretty clear and cut and dry. Also it's a film, you are expected to fill in the blanks and not ask retarded questions. There isn't enough run time to spoon feed you.

First off, Miki moved elsewhere sometime during those five years and they stopped seeing each other on a daily basis. Secondly, what Taki and Mitsuha felt was a strong sense of longing for something, although they didn't know who or what. You don't naturally feel like diverting your attention to something else in those situations.
