In such a large structure, it's more than likely that there are more humans eking out existences in other places. From the movie alone we know that there are plenty of places that the safeguard doesn't know about or can't access.

Considering how fucking massive the megastructure is and how small a distance it’s possible for humans to travel in a lifetime, wouldn’t humanity only take up a very minuscule part of the structure and the rest is just endless buildings with no life?

Killy succeeds in the first part of his quest, after the child is grown, Killy takes him back to the city to solve everything out, that's why at the final panel you see Killy and behind him a small person wearing a hazmat suit so the disease or wathever is infecting humans, don't afect this child
