Marco loved his best friends fiancee. His friend died right after he was married and he felt guilty for it, as if he should have died instead. This guilt festered until it actually became visible on the outside of his body (symbolism).

Then the rest of the movie is mostly just a series of fun events that got him to loosen up and gain the willpower to tell Julia how he really feels. Like you said, letting go. It is implied that once he came to this decision, he stopped looking like a pig. But they never actually show it.

I really hope not. There's no room to make one with the ending the movie got. Unless it focused entirely on Fio or Curtis. And then who would want a movie focused on them (aside from eye candy with Fio).
