First time through I make as close to me as I can and usually idealized a bit
Second time through I'll make a fap bait waifu

Usually male, but every once in a while I'll go female, or do a female run if it's different or has cute outfits.

Usually Male Dragon Age Inquisition was the only game I went female and it was shit.

Almost always female, unless it is an online game that I will have to interact with other people a lot.
I still make female characters in online games, just ones where I won't have to get into a skype/discord/teamspeak call with them because we're about to do a boss or something, and they find out I'm a dude.
Almost every opportunity I get, I will make my character a female. If I can't do it in real life, I'll do it in a video game.
