Well not really illusions, more like modifications. Yen defiantly did modify herself to make her look "perfect". Triss on the other hand was burned at the battle Battle of Sodden Hill to the point that she wasn't recognizable and was actually miss accounted as being dead. She was healed via magic.

Why would Geralt want his scars healed? He's not a sorceress where it's literally required for them to look good in the North. Plus the bitches love it. Yen gets wet in the books when see shes him shirtless for the first time and sees the scars on his chest.
