It isn't explicitly stated anywhere, only implied by in game dialog. After Overwatch HQ blew up due to Morrison (76) and Reyes (Reaper) fighting, Mercy attempted to resurrect Reyes but half-failed. The official site says this put him into this weird undead state of his cells constantly regenerating at the same rate the decay.

this could be just mercy not wanting to kill anyone though.
if mercy asks reaper "what has happened to you?" he replies with "you tell me, doc"
this could just be reaper making fun of mercy for being a doctor
and when reaper kills mercy he goes "dont forget, you caused this"
which might be that he hates mercy for fucking up a surgery or something, but it could just be referencing his reason for the betrayal of overwatch
