Tower of Latria is a bit of a joke on repeat playthroughs, but yeah nothing in Bloodborne really manages to capture the dread of that first playthrough.
Honestly I'm not even sure they tried, the only "fuck you" enemy in Bloodborne is the Winter Lantern and any dread is quickly replaced with annoyance at that line of sight crap.

I'd go with the later, but my main reason for the change would be to have the world tendency not be some arbitrary thing that made the game harder or easier just because you failed this or killed that. I'd make it something that uncovered the dark nature of a level and therefore revealed a hidden route or greater evil hiding within.

If you lose however, you have to repeat a challenge to which you now know the solution, so by increasing the difficulty via tendency, you bring the challenge of this previously done challenge closer to its original level.
