I dunno, look away every 20 minutes to a distant object to give your eyes something to focus on thats farther away to reduce eye strain.
Wrists, keep them level with your keyboard and floor, reduce keyboard elevation, don't always hover them over the keyboard.

Take a break every hour. Go outside and look at shit from varying distances for a couple minutes instead of a screen all day.

Even still you might be sitting on the couch slouching - which weakens the core and forces your back muscles to take up the work when you are doing activity and leads to back pain
Or your neck might be in the wrong position and leads to soreness and neck pain in the future.
I don't have any graphics for proper couch sitting though.
Funny thing is for my pirriformis syndrome, sitting on a couch or any soft cushioned seat is an invitation to instant pain in my hip and legs. So not everyone has that luxury.

Just like you are supposed to do with sitting posture-
Alternate from 90 degrees, to 135 degrees to somewhere in between, every half hour or so, and get up every half hour too, walk around a bit and then sit down.
Moderation and shit, the important thing is to not put too much pressure on your disks for too long a time - like say sitting at 135 degree angle for 3 hours, instead of sitting at 135 then changing it to 90 after an hour or so.

the back pain though, i've had since before i got super fat, super fat by my standards, i used to be a seal candidate and in military working on spec ops base, and like i could out PT everyone in my entire ship easily and even still now when i work out i lift 300+ lbs regularly while doing like 100 reps
but man, no matter how much weight i was, i always had back pain, just always. even when i was under weight, its probably due to OP because i am always at my computer like 24/7 even when i was in the military i had a laptop and would sit there every night playing new game releases
Don't worry. Your eyes won't get ruined just by being close to the monitor or looking at it for extended periods of time.

For wrists, make sure
1. you have a good keyboard, a shit keyboard can fuck up your wrists more than you think
2. your elbows are bent around 90 degrees while typing, so your wrists are straight, make sure your chair is adjusted to the correct height to allow this, or get a proper desk
3. you do NOT type with what they teach as "perfect typing form" with your fingers planted on the home row 24/7 and such. just freestyle it when typing for good hand and wrist health, keeping your wrists and fingers in static positions for too long will fuck your shit up

Mine diagnosed my L5-S1 herniation, no back pain though, its all in the hip and legs/feet

nah i have like what doctor described, semi-flat feet. like no matter the amount of work im doing, just like after X amount of hours my back just fucking goes to shit and im in extreme agony , like if you've ever been in so much pain that your sweating profusely and have to take a knee and feel like your going to throw up, thats how i felt every day at the job i tried to work at and now how i feel if i like even try to jog for a minute

When eyes start deteriorating, they deteriorate faster over time. They already are very deteriorated in your case. In two years, 3 if you're lucky, you won't be able to recognise faces up to 5 meters (16 foot) or differenciate humans / pole at night if they are farther away than 30 meters (98 foot)
Go seek medical help. Even if you never go out, which is without a doubt the case (if it wasn't you'd have told me how you can't read signs in the streets instead of talking about your TV), you'll get to a point where everything on your monitor will be blurry.
And it won't take that long in your case.

I know it was using a smartphone in bed at night. I used to have great vision, but then I started reading shit on a 4" screen in the dark. Fucking destroyed my eyes totally.

Oh please, don't get me wrong by believing I think we should never discuss the good hobby that we all share, I just personally believe that Sup Forums exists in its greatest form when it forgets this need to follow a rigid existence of self-flagellating self-loathing fury directed at one another over opinions.
I've had a lot of pretty good threads on Sup Forums, both about and with little to do with video games. They do best when Sup Forums forgets to hate itself for its failures or self-conciousness. I am simply suggesting it let itself go, relax the belt buckles a bit and enjoy life.
