I just assumed her death was scripted.
Felt bad when I realized my mistake.

It was my first DX game. I didn't think about it as this game where they'd give you that much choice over the main plot

There's not really any other point in HR that gives you that kind of option to influence the plot without telegraphing it ridiculously. It's a nice easter egg.
