Even among fanservice games, why is fetish DLC so rare? It seems like a genius idea for DLC, if it doesn't require a great deal of effort and said fetish isn't incredibly obscure. I bought pic related for $5 and can only imagine how many others have done the same.

Maybe devs still feel it's too harsh for the general public. I wouldn't be amazed if fetish DLC's started popping up in the nearby future, after the makers notice that stuff like that's steadily getting more acceptable.

Well the way I see it is that they want to appeal to the most people to get the most money, would you want to spend a good few hours/days working on something you wouldnt make your money back on?
If one of the selling points of your game is the attractive characters, creating extra content for those with unique preferences is not a bad idea.

Feet, armpits, huge cowtits/breast expansion/flat chests, THIQQ (simply adding a slider for these last two cases as DLC, intentionally leaving it out of the main game would net loads of money), and crossdressing, off of the top of my head, are all relatively common save armpits and not explicit enough for Valve/Sony/Nintendo to have an issue.

Akiba's Trip had really great cross dressing. It really helped that you could unlock a whole bunch of animation sets for the main character. Too bad the combat is so clunky. There were a lot of times when I'd activate my special but it had no hitbox.
