That's literally possible with this. You can make a male frame that likes to wear masculine clothing sit down to pee and get pregnant.

You can make a female frame that likes to wear feminine clothing stand up to pee and get others pregnant.

All of these gender options are on a per-Sim basis, so you can create your own futa harem in-game.

This almost makes me want to pirate it

I really don't see what's wrong with this. Sure, "Can get pregnant/can pee standing up" is unrealistic, but it just gives more options, and that's always a good thing, even if they're options I'd never use.
