I do remember playing PSO2 with some random japs right when super hard advance quests first came out and we killed just one small fry monster before one of the randoms picked up an 11* weapon, which at the time were the shit. Less than 3 seconds had elapsed. Could not believe it.

Depends on the mmo and the mob, I'm currently camping a rare item, and it's as high as 15-20%. But theres only 10 mobs or so and they have a gigantic respawn time. And are also former raid trash. So it's still annoying as fuck to do.
