They did, the first patch for GZ was literally those missions as free DLC.
Kojima's name was removed from the cover as a professional courtesy, to give him exclusive control of his "a Hideo Kojima game by Hideo Kojima" tagline shit. Konami doesn't give a shit how many times his name appears on the screen ingame, they just won't use it to sell products.

Some people have download limits, or slow enough speeds that downloading for 2 or 3 days is a reality. I download everything myself, but I still think it's lazy as fuck to ship a disc with a steam installer, as well as dishonest for not clearly advertising it as that.
