You come to Yharnam to cure some disease because Yharnam blood is famous for curing people.
In return, you are contracted as hunter to go out and kill some beasts.
In past times, Yharnam worshipped old gods and sought a form of enlightenment from them.
They wound up discovering old gods existed, and managed to communicate with them. Old gods can't procreate by themselves, and thus used humans in an attempt to do that.
One such child, Mergo, was the source of the current nightmare afflicting Yharnam.
After going about, killing beasts, madmen, aliens, and alien children, you return back to the hunter's dream. At that point, you're given the option of fighting the Moonlight Presence, who is implied to have been using you to go out and kill other Old Gods (such as Mergo), and if you have accumulated enough insight through the consumption of the one third umbilical chords, you can become an infant Old God yourself.
