Pain-staking detail. Injuries down to the fingerbones are tracked. Enemies do not die of "arrows" or "swords", they die of blood loss, asphyxiation, or brain trauma. Someone who gets punched regularly looses teeth, and you can track where they go. You can punch an elephant in the leg a million times, and it will die of starvation or dehydration before it dies because of you punching it.

In Adventure Mode you can create a character who specialises in wrestling and grab enemies by any body part, rip it off and stab them with it. Want to gouge out your enemy's eyes? Go for it, then they'll wander around blindly trying to find you while bleeding everywhere. Maybe then you'll want to choke them to death. Or maybe grab them by the leg and throw them at another enemy.

Wow. Just wow. The amount of freedom in the combat reminds me of table top rpgs like gamma world and d&d.

You do realize even in real life you can survive headshots, it all depends on what part of the brain is damaged.

It's pretty ridiculous. Some methods can be pretty slow to get anywhere, but it's still ridiculous what can happen. Also, you can pick up and throw just about anything, so you can literally Pocket Sand people. It won't necessarily do much of anything, but you still can.
Oh, and you can drink the blood of your enemies instead of water to stay hydrated.
